REQUIREMENTS TO PROVIDE A 232 PURSUANT TO 223(f) LOAN ANALYSIS FOR THE ACQUISITION OR REFINANCE OF EXISTING
NURSING HOME & ASSISTED LIVING FACILITIES

 

If you provide AMERICAN MORTGAGE with the following preliminary information, we will be pleased to provide you with a mortgage proposal which will outline the level, terms, and conditions of the financing we can arrange for you:

  1. Project description.
  2. Site location map.
  3. Annual operating statements for each of the past 3 years, and year to date.
  4. If refinance; summary of existing debt and copy of mortgage(s) and note(s).
  5. If acquisition; copy of option or purchase contract.
  6. Construction budget (or) list of proposed repairs and cost estimate.
  7. Proposed list and cost estimate of major movable equipment to be replaced.
  8. Resume on proposed sponsor and management agent.
  9. Market study or appraisal (if available).
  10. Survey, site plan, or architectural drawings (if available).
  11. Copy of Certificate-Of-Need and/or Nursing Home License (if applicable).
  12. Lease or Management Agreement (if applicable).
  13. Project bed mix including unit sizes, number of bedrooms, number of bathrooms, and bed rates.
  14. Operating expenses (detailed).
  15. List of any proposed ancillary income detailing the source and anticipated monthly revenue.
  16. Current census.
  17. Occupancy history for most recent year and each of the last six months.
  18. List of project amenities and what is included in bed rates versus what is paid for by tenants.
  19. Most recent Department of Health Inspection report, if applicable.
